I'm going to start this thread off.

I don't use the same email address for any forum or webpage.
That being said, I need my "username" attached to my "migrated acct"

To explain, I would use something like Swype.com@MyDomain.net for this forum & I would use GPONA.com@MyDomain.net for http://www.gpona.com/forums
But I would use FastKatt as a username if it was available.
Clear as mud?

So, what is the best way for me to connect my old self with my new self?

The ability to change your Username and Email Address is included in the new forums. Simply click the "Settings" link in the top-right corner, then click "Edit Profile" on the left. You'll information that page both for changing your email address and for changing your username.

So, what you would do is follow the instructions in the forum notice at the top of every page (visible when logged out) for recovering your migrated account. Then, change your username and email address as you see fit.

If you do not have access to the email account you registered for UserVoice with, but can provide appropriate identifying information, you can send me a Private Message with the necessary info and I'll manually fix your accounts. I get grumpy when I do that too often though :)
